Marc Glisse f5e4857849 gmpxx 4.1 and 4.2 don't have the same declaration for mpz_class, so we 
cannot forward declare it ourselves.

Joachim Reichel f582718758 rename create_makefile to cgal_create_makefile to avoid name clashes, 
adjust all occurences of create_makefile and create\_makefile in the entire
