Removing leading and trailing empty lines from all example files.

Using the following Perl script:

Actually, the "// file: example_bla.cpp" lines are not needed anymore,
...
since the manual tools know it. So let's just remove them (many were
not uptodate with the filemane anyway).

adjust all occurences of create_makefile and create\_makefile in the entire
repository (well, trunk only)
