Sylvain Pion 2d42164092 - New Handle_for_virtual<> and Ref_counted_virtual. 
- Ref_counted is not required anymore.
- Remove all Simple_* rep classes as deriving from Ref_counted is not
  necessary anymore.
- basic.h cleanup.
